Please send all grant application documents in pdf format by email to:
The grant request must include the items listed below.
Items 1-9 must be on letterhead.
1) The official name and address of your nonprofit organization
2) IRS EIN# of your nonprofit organization
3) Your name, email address, and phone number
4) A brief history of the organization
5) Names and affiliations of the organization's Board of Directors
6) The purpose, project, or activity description - Please include:
7) Detailed description of how the grant funds will be utilized and the timeline involved
8) Current and prospective major funding sources for the organization and the project
9) The grant request should be signed by the officer authorized to make the request on behalf of the nonprofit.
10) The organization's tax exempt letter
11) After the grant funds are spent, submit documentation as to how grant funds were utilized. This can be in writing with or without photos as appropriate.
