Grants must be for a specific project or activity. Please note that the OMF will not fund a grant that incudes staff salaries.
A grant request must include the following items, on organization letterhead (other than item 7), and be signed by an officer authorized to make the request on behalf of the organization:
About you and your organization:
1. Your name, email address, and phone number;
2. The official name, street and mailing address, and website
address (if applicable) of your nonprofit organization;
3. The IRS EIN# of your organization;
4. Names and affiliations of your organization’s Board of
Directors;
5. A brief history of your organization;
6. Current and prospective major funding sources for the
organization;
7. A copy of your organization’s tax exemption letter.
About the grant request:
1. The project or activity description, including:
a. The need(s) addressed;
b. The outcome(s) sought;
c. The means by which the outcomes will be achieved.
2. A detailed description of how the funds will be utilized and
the timeline involved. Please note the OMF prohibition on
the payment of staff salaries with grant funds.
3. Current and prospective major funding sources for the
project/activity.
After the grant funds are spent, please submit documentation on how
grant funds were utilized. This can be in writing with or without
photos as appropriate.

After we receive your application, we will review it. If the OMF is
interested in funding the grant, we may request additional information
about your organization or project.
